A very important part of the work we do is to carefully help our young people through the transition from a residential environment to that of a Foster family.We work with multiple agencies to ensure that a full overview of the young person’s needs is discussed.The young person has the chance to decide what family they wish to part of, and they can undertake visits to see the home and the family – spending time together and getting to know each other. Initially they are supported by their keyworker and then to overnight visits and week visits until they move permanently. The foster family are also encouraged to come and see the young person in their own environment where they are being supported by people they already know and trust.For each young person that we support in this way we create a detailed transition plan – this combined with multiple visits helps reduce the chance of placement breakdown.Next week AP is making the transition to live with her new foster family after being with us for 3 years and we wish her all the luck in the world.
